Use social proof, particularly with popular logos wherever you can, it's great for conversions in that it reduces anxiety of giving your email
Test just using an email for your CTA form - it could very well convert better than asking for the person's name as well & today, people are more savvy to email marketing so using their name in your email templates doesn't make as big a difference anymore.
