Appsumo uses three email opt-in forms in their posts. One at the very beginning (often called a welcome mat or gate), one in the sidebar that fades in upon scrolling into view and one that pops up in the bottom right corner once you scroll to the end of the page.
Notice how each of these forms, all displayed & created differently, are shown one at a time as you scroll. None of them ever compete with each other.
What's great about this approach is that there are many opportunities created to get a subscriber and instead of seeing the same form over & over again, you see a variety of different forms & copy which resets your attention.
